Cost per Natural Cycle: $2,085
- All services not rendered are refundable.
- Patients with insurance coverage documented by written authorization will pay only a deposit.
- The amount of the deposit will be determined on an individual basis during a financial consultation.

Cost per Programmed Cycle: $2,620
- These fees are due in full two (2) weeks prior to start of stimulation medications.
- All services not rendered are refundable.
- Patients with insurance coverage documented by written authorization will pay only a deposit.
- The amount of the deposit will be determined on an individual basis during a financial consultation.

All fees are subject to change without advance notice. If the couple or individual and PFC have executed a financial agreement, the price stated in the agreement will be honored for the cycle, regardless of any change in fees occurring after the document was signed. Prices effective 11/01/08. |
